Curtis V. Cooper Primary Health Care is happy to offer medical services to the citizens of Pembroke, GA and surrounding cities. Adults and children are welcome at this location. We accept most insurances and we have a sliding fee discount program for uninsured and underinsured patients. Services include but not limited to chronic care management, women's health, physical, wellness checkups, immunization, COVID-19 screening and vaccination, etc.
